Step 1 - Navigate
Currently, there are two places you can print a claim form:
The Claims Manager
The Claims Queue
Claim forms are generated by the Clearinghouse, so the claim MUST have either
been Billed electronically, or
been Printed from the Claims Queue
Step 2 - Print
Click the print icon (Claims Manager - see below) or the "Print" button (claims queue).
Then make your selections and Print Paper Claim
